We are seeking a talented and motivated Intake Coordinator to join our Manhattan, NY office team.

The Intake Coordinator requires skill in planning, organizing and coordinating the delivery of patient care at the point of the patient s referral. The Intake Coordinator has direct contact with referring physicians, hospital liaisons and patient/families as it relates the delivery of patient care at the point of a patient s referral.
Gathers pertinent information about the referred patient to include demographic, clinical and payer source information and completes written referral form.
Obtains the official physician s order for start of care. Obtain and input clinical information.
Provides the Billing Department with complete and accurate insurance information and authorization. Check verify member eligibility.
Communicates with the Director of Nursing and Assistant Director of Nursing any information received from the physician, patient or employees about the patient s care, needs or accepting the referral.
Keeps an intake log of all referrals with outcomes.
Records all incoming calls.
Inbound/Outbound calling, Transferring, Customer Service, Data Processing/Updating and Phone call processing.
Assist with scheduling as needed.
Cooperate with Medicare, Medicaid and private insurance providers to resolve billing issues.
Organize files, developed spreadsheets, faxed reports and scanned documents.
Helps with creating methods to better help with understanding process methods using medical knowledge and medical criteria based on company determination with approval of radiology procedures.
Performs additional duties assigned
Knowledge of CPT and ICD-10 codes (required)
Intake Coordination Experience (required)
Managed Long Term Care (MLTC) experience (required)
Experience in Home Care Agency environment (required)
Experience with HHAExchange software (preferred)
Proficiency with computers, Microsoft Office Suite, internet, etc.
Bachelor s Degree (preferred)
Bilingual English/Spanish (preferred)
